The Fourth Wave: Digital Health Newsletter for Apr 13

In response to Facebook CEO Mark Zuckerberg’s testimony this week before the United States Congress (C-SPAN video here), U.S. Senator Bernie Sanders (I-Vt) posted a video response highlighting Facebook’s role in our democracy and the most important questions we should be asking concerning Zuckerberg’s testimony. The video is viewable here and the full tanscript is as follows:

“Today, Mark Zuckerberg will testify before the Senate and answer questions about Facebook’s role in our democracy. This is a good first step. Clearly, among other things, we must end the epidemic of fraudulent political advertising that currently exists. But that is nowhere near enough. Today’s hearing must be the beginning, not the end, of the discussion about the dramatic impact Facebook, technology companies and the internet at large play in our society.

While the internet has been an enormous source for good, all that convenience and connection comes with a price. Our privacy has been invaded in an unprecedented way and in a manner that would have been unthinkable even 20 years ago. Companies today know every aspect of a person’s life—where they go, what they do, their medical and financial records, whom they talk to, what they buy—and use that information to make billions in profit.

This nation needs a serious conversation about the proper role of these companies. Should Facebook, Google and Amazon be allowed to control so much of the internet? How do ordinary citizens benefit from the internet without seeing an invasion of their personal lives? With the enormous power of Google, Facebook and Amazon, how do we maintain the diversity of opinion that a vibrant democracy requires?

This is an issue that impacts virtually every American. The American people have a right to know how these companies are profiting from their information.”

— U.S. Senator Bernie Sanders (I-Vt)

LIVING AND SOCIETY

Uber has introduced new passenger safety features, including an option to identify trusted contacts who will be automatically notified about trip arrivals, and a one-tap 911/emergency assistance button. The company also plans to perfomr annual background checks on its drivers.

As a result of President Trump signing laws cracking down on websites used by sex workers to connect with and screen clients, Australian social impact startup Assembly Four created Switter, a ” Twitter for sex workers” that’s part of Mastodon, a decentralized social networking platform.

REGULATION AND REGULATORY APPROVALS

The FDA has released three new guidances on Next Generation Sequencing (NGS) and in vitro diagnostic development. Commissioner Scott Gottlieb’s comments are located here: “Leveraging Innovation for the Treatment of Cancer

The FDA has approved IDx’s AI device to detect diabetic eye disease. The system provides a screening decision without the need for a clinician to also interpret the image or results.

The FDA has issued a draft guidance intended to provide device companies with a more flexible iterative approach in applying for 510(k) clearances. FDA Commissioner Scott Gottlieb states that this will: “modernize the FDA’s approach to moderate risk devices by allowing manufacturers to use objective performance criteria to facilitate demonstration of substantial equivalence of their new products to legally marketed devices,” The new option also doesn’t require direct comparison to a predicate device as part of a premarket submission.

According to Bradley Merrill Thompson, a member of the firm Epstein Becker & Green, P.C., the FDA has reneged on its promise to take a risk-based approach to CDS software. As Brad writes: By law, FDA is supposed to focus its limited resources on higher risk technologies both to maximize public health protection, and at the same time to avoid stifling innovation where regulation is unnecessary. Unfortunately, in developing its new approach to regulating clinical decision support (CDS) software, FDA has chosen to ignore that basic principle as well as a promise the agency made to Congress and the public.”

WEARABLE TECH

A woman’s Apple Watch helped detect her severe thyroid problem. As Caroline Kee at BuzzFeed News reports, “Heather Hendershot had no symptoms and felt fine — but her Apple Watch indicated that she might have a health problem… (it) detected an unusually rapid heartbeat… and began to beep. The device had detected that she had a resting heart rate of 120 beats per minute, which is abnormally high for a young adult resting on the couch. The “normal” resting heart rate range for adults is 60 to 100 beats per minute.” After visiting a clinic and then going to the emergency room, a blood test resulted in a diagnosis of severe hyperthyroidism.

Misfit has launched a new slimmed-down hybrid watch called Path. The wearable comes with a lot of the usual fitness features and is waterproof, but it isn’t rechargeable. The cell battery is replacable and lasts for up to six months.

According to an FCC filing, Snapchat will give wearables another shot, as it plans to introduce a new version of its Spectacles Wi-Fi camera glasses. About 150,000 units of the first version were sold and the company had to take a $40 million writedown as a result of the dismal sales.

GENOMICS

study published in bioRxiv indicates that: “For the first time, a complete genome of an RNA virus has been sequenced in its original form. Previously, RNA was sequenced by the chemical degradation of radiolabelled RNA, a difficult method that produced only short sequences. Instead, RNA has usually been sequenced indirectly by copying it into cDNA, which is often amplified to dsDNA by PCR and subsequently analyzed using a variety of DNA sequencing methods. We designed an adapter to short highly conserved termi of the influenza virus genome to target the (-) sense RNA into a protein nanopore on the Oxford Nanopore MinION sequencing platform.”

Thirteen European countries have signed an agreement to deliver cross-border access to their genomic information. This is reportedly a game changer for European health research and clinical practice, as sharing more genomic data will improve understanding and prevention of disease, allowing for more personalized treatments (and targeted drug prescription), in particular for rare diseases, cancer and brain related diseases.

James Gallagher, presenter of The Second Genome on BBC, writes that human cells make up only 43% of the body’s total cell count. The rest are microscopic colonists, i.e. our microbiome. James adds that “Understanding this hidden half of ourselves is rapidly transforming understanding of diseases from allergy to Parkinson’s.” Prof Rob Knight from UC San Diego adds that: “It’s incredible to think each teaspoon of your stool contains more data in the DNA of those microbes than it would take literally a ton of DVDs to store.”

FUNDING AND INVESTMENT

Kaiku Health Oy, which provides intelligent patient monitoring software for healthcare providers across Europe, has closed a €4.4 million round of funding.

Livongo Health has raised $105 million and is partnering with Cambia Health Solutions to improve health for people with chronic conditions nationwide.
